Active treatment concept for chronic neck pain
Goal: active instead of passive
Instead of passive treatment of the painful tissue (joints, muscles, connective tissue), the path to active strengthening, mobilization and Coordination training the muscles, as well Improvement of general physical fitness go. In the case of severe pain and fear of movement, it makes sense to enable activity to be taken up through passive physiotherapeutic treatment and medication and gradually taper off as the disease progresses.

The spine is difficult to treat. On the one hand it is exposed to high mechanical loads, on the other hand it has great mobility.
The treatment of the spine (e.g. herniated disc, facet syndrome, foramen stenosis, etc.) therefore requires a lot of experience.
I focus on a wide variety of diseases of the spine.
The aim of any treatment is treatment without surgery.
Which therapy achieves the best results in the long term can only be determined after looking at all of the information (Examination, X-ray, ultrasound, MRI, etc.) be assessed.

Preliminary examinations
Before the active physiotherapy is started in the group, differential diagnosis should be carried out to ensure that it is really chronic neck pain without additional, otherwise treatable causes of pain.
In practice, unfortunately, it looks like that many patients with Neck pain sent to group treatment from the outset without targeted diagnostics and treatment for cost reasons. Often, acute functional or structural causes of pain are not treated, which reduce the success of the treatment or even lead to increased pain during activity, to which those affected react with an increased fear of movement.
These could be:
- acute dysfunction of the cervical vertebrae
- acute dysfunction of the Thoracic spine or the shoulder joints
- "High tension" in the neck - and Shoulder muscles
- Condition after whiplash
- Disc protrusion (protrusion) or disc prolapse (Nucleus pulposus prolapse)
- Arthrotic changes in the cervical spine (osteochondrosis)
- Constrictions in the spinal canal (Spinal stenosis of the cervical spine) or at the nerve exit holes
- inflammatory changes in the small vertebral joints (Facet syndrome)
Individual therapy versus group therapy
Studies have shown the effectiveness of physical therapy treatments chronic neck pain checked. For this purpose, the passive treatment of chronic neck pain with massage, manual therapy and physiotherapy in individual therapy was compared with the consistent implementation of a fixed exercise program in a closed group of 6-8 participants over a certain period of time. After completion of the respective treatment measure, the participants were asked about their perception of the impairment caused by their pain, the frequency of absence from work due to their complaints and their satisfaction with the treatment measure carried out. The result was amazing. Active group therapy, in particular strength training / muscle building, was more successful and sustainable than individual treatment.
Neck school
Physiotherapy practices offer so-called under professional guidance Neck schools in which on the special problems of the chronic neck and Headache is received. A closed group concept with 6-10 participants over a period of approx. 10 weeks makes sense. If a corresponding group offer is not available, information and training can also take place in individual therapy (more expensive).
The group has the advantage that you can exchange ideas with other sufferers and see that you are not alone with your pain. In addition, the not to be mistaken "Fun and motivational factor“Certainly higher in the group. Fun and joy in movement support the healing process and the "overcoming the bastard" when continuing the program at home.
It is also advisable to take part in a preventive (preventive) group in a neck school if there are no or only latent symptoms. Most health insurance companies cover around 80% of the costs as part of their prevention offers. Another option is to take part in 50 units of rehabilitation sports that can be prescribed by a doctor.
